Professionals,
We can't wait to meet your young adult! Maybe they have one month or six months to go before emancipation- that's okay! We understand the importance of the young adult having an opportunity to feel comfortable and secure in the decisions that they are making.
Our referral form helps you paint an important picture of your young adult. This is your opportunity to tell us about them, and what makes them unique. As we get the opportunity to begin learning about them, it is important to share the whole picture. Some young adults are a great fit for us, and some are not- and that's okay! We are here to help!

Referral Form
Once we receive your Referral Form, we will reach out to schedule a tour of our house. During this tour your YA will get to see what an apartment looks like, and get a feel for what life would be life for them. They will meet their team which includes the House Parents, the Executive Director, and an After-Emancipation Liaison if they don't already have one.
